"Most DOT exams are performed by medical professionals who have never been on a worksite at 6:30 AM. As a former CalFire firefighter and EMT, I spent years responding to exactly the kinds of hazards and medical emergencies CDL drivers face every day. I built Hamilton Mobile Chiropractic to bring that first-responder discipline — speed, precision, and absolute regulatory accuracy — to your yard."

Frequently Asked Questions

A DOT physical is a federally mandated medical examination under 49 CFR §391.41 required for anyone operating a commercial motor vehicle (CMV) with a gross vehicle weight rating over 10,001 lbs, transporting placardable quantities of hazardous materials, or carrying 8 or more passengers for compensation (16 or more without compensation). Most drivers require a renewed certificate every 24 months. Drivers with certain controlled conditions, such as managed hypertension, may receive 1-year or shorter-duration certificates. If your drivers hold a CDL or operate vehicles in these categories, they need a current Medical Examiner's Certificate on file.

Yes, literally. The entire clinical setup (Seca stadiometer, Titmus V4 vision screener, automated sphygmomanometer, ADC diagnostic set, and CLIA-waived urinalysis strips) packs into a ruggedized Pelican case. We have performed exams in logistics yards, construction staging areas, and municipal maintenance facilities throughout North County San Diego.

An individual exam takes 25 to 30 minutes, including intake review and urinalysis. For multi-driver visits, we typically complete 8 to 12 drivers in a 4-hour morning block. Drivers who complete the digital health history on their phone before the scheduled exam save approximately 15 minutes per driver.

Paper-certificate requirements may change as California and FMCSA update electronic reporting. We submit exam results to the National Registry and provide drivers with the documentation required under the rules in effect on the examination date.

Yes. California Assembly Bill 2143 amended Business and Professions Code §1206.5 to explicitly permit chiropractors listed on the NRCME to perform urine specific gravity, protein, blood, and glucose testing for DOT exams. This means no separate lab trip or additional scheduling.

Drivers should bring a government-issued photo ID, their CDL or learner's permit, a list of all current medications (prescription, OTC, and supplements), glasses or contacts if used, hearing aids if used, a CPAP compliance report showing usage data if prescribed, the MCSA-5870 form completed by their treating physician if insulin-dependent diabetic, and any recent specialist clearance letters from a cardiologist, neurologist, or other relevant specialist. Drivers who complete the digital intake form before the exam will have already captured most of this information.

A single-driver mobile exam is $175 flat. Multi-driver pricing starts at $125 per driver for 3 or more drivers at a single visit. The employer loses approximately $90 in driver wages due to round-trip travel and wait time, making our mobile exam mathematically cheaper once downtime is counted. For programs with 10 or more drivers on a recurring annual or bi-annual schedule, call (619) 323-5020 for special pricing.

Depending on the disqualifying condition, the driver may be eligible for a temporary short-duration certificate (for example, 3 months for Stage 2 hypertension), a specialist clearance pathway, or a federal exemption program for seizure, hearing, or vision conditions. Dr. Hamilton will explain the next steps in plain language and provide the exact documentation needed for your driver and HR team.
